After the ACA became law, health providers are legally responsible for the patients health after a procedure.
Not following up with a patient can lead to all sorts of issues that ultimately cost everyone in the insurance pool more money. This application is aimed at helping health providers engage with patients after their procedures and be able to monitor and identify patients in most need of engagement or intervention.
Understanding the eco-system
My team was tasked with only the health provider side of the equation, however to better design for this we needed to understand how all the touchpoints and systems orchestrated into a customer journey.
How will this help?
Behavioral change is probably the hardest thing to accomplish with users of any kind, even more so when it comes to getting folks to do the little things that add up over time and don’t extract en immediate reward. This system will help people connect with people at hopefully the right times and create a seamless user experience where patients feel supported the entire way.
